Nurse Practitioner

This position will be for a Nurse Practitioner seeing patients in Skilled Nursing Facilities as well as Assisted Living Facilities.

The Nurse Practitioner (NP) will provide advanced patient care, including assessments, diagnosis, treatment, and prescribing medications. The NP collaborates with physicians, nurses, and other healthcare professionals to ensure high-quality care.

Key Responsibilities:

✔ Conduct physical exams, diagnose conditions, and develop treatment plans.

✔ Order and interpret diagnostic tests (labs, imaging, etc.).

✔ Prescribe medications and treatments per NY state regulations.

✔ Educate patients on health conditions and preventive care.

✔ Work closely with physicians and healthcare teams.

✔ Maintain accurate documentation in EHR systems.

*Education: Master’s or Doctorate in Nursing (MSN/DNP). *Certification: AANP or ANCC board certification. *Licensure: Active NP license in New York. *Experience: 1-2 years preferred
